At level one you choose which old god you wish to control, each old god has their own trait and set of abilities.

Power of the Old Gods
You replace the core. You have 21000 (+600 per level) health, but have no shields, when below 50% and not taking damage regain 100 (+2.86 per minute) each second . Old gods also have a 75% resistance to percentage damage.
Instead of firing lasers at nearby enemies you instead summon up to 3 tentacles that deal 300 damage per second (each tentacle can only attack one target, only one tentacle per target.)

Each of C'thun's basic abilities has a quest that empowers his heroic.

An Old God's embrace
Whenever you use an ability you give your teamates for 20 (+4% per level) shields, up to 200 (+4% per level).

Corrupted Blood
Enemy heroes damaged by Yogg-Saron will take 90 dot (+4% per level) over 4 seconds. This Dot will spread to nearby heroes after 3 seconds, Corrupted Blood spreads indefinitely.

For each summon you have active you grant you team +2 armour and +2% increased healing. Stacks up to 4 times

Summon Eye of C'thun
Replace a friendly minion with an Eye of C'thun. This Eye grants vision, has 500 (+4% per level) health and deals 40 (+4% per level) damage (The eye attacks 3 times per second). This eye moves at 120% speed. 15 second cooldown (starts once the eye is killed)
When the eye is being controlled by C'thun it gains the ability to fire an Eye Beam which does 0.75% of the targets health as damage once a second.
Quest: For every 5 seconds of hero vision granted reduce C'thun's max heroic cooldown by 1. (If the hero could already be viewed, grant no stacks of quest)

Summon Qiraji
Summon a party of 4 Qiraji to either push a lane, or assist a hero in combat. Each party consists of: one Qiraji gladiator, one Qiraji prophets, and two Qiraji emperor. lasts 40 seconds. 60 second cooldown.
Qiraji gladiator: 2500 Health, 40 damage. 50% Lifesteal Qiraji prophets: 1500 Health, 60 damage. Silences Minions and heroes for .75 seconds. Qiraji emperor: 2000 Health, 40 damage. Damaging an enemy improves the emperor's damage by 5 up to 80, lasts 3 seconds.
Quest: Minion kills improve C'thun's heroic damage by 2, hero kills improve C'thun's heroic damage by 8

C'Thun's Blessing
Grant a teammate your blessing, giving them 250 (+4% per level) bonus health, 10% increased damage and movement speed for 20 seconds, but also reducing armour all healing by 10%. 20 second cooldown.
Quest: For every 3 kills fire another bolt from c'thun's ult.

Summon Naga
Target a friendly hero, summoning a naga to heal their wounds. 15 second cooldown (Starts when no Naga are alive
Naga have 800 (+4 per level) health and heal 200 health over time to the primary hero, and 125 to heroes near them once every 10 seconds.

Tentacle of N'Zoth
Extend your reach. You now control one of your tentacles, while it is not attacking/ using abilities it is buried underground untargetable (w/ a visual similar to thralls sundering showing where it is). You can use this tentacle to aid your allies further.
Q- Thrash: Thrash out in an arc at your enemies, dealing 80 (+4% per level) damage, enemies hit have their healing reduced by 6% for 5 seconds, stacking up to 30. 3 second cooldown. W- Dark Presence: Reduce nearby enemies armour by 5 for 12 seconds. 30 second cooldown (Half the area of tormented souls) E- Constrict: Constrict an enemy, rooting them for 1 second. 8 second cooldown

Herald of N'zoth
Select a friendly hero to be your herald. Grant them a 10% buff to all stats and 6% life steal for 15 seconds. 30 second cooldown.

Spawn Faceless one
Spawn a faceless one from your essence (one can be active at a time). Faceless ones have 800 (+4 per level) health and deal 120 damage. 10 second cooldown. (Starts after the Faceless One has been killed)
Faceless ones are spawned at a selected friendly hero, activate to control the Faceless one.

Raise Forgotten One
Raise a forgotten one, it has 400 (+4% per level) health, and spews out a jet of flame once every 6 seconds which deals 180 (+4 per level) damage. It cannot move, but it also cannot be damaged by abilities.

Empower Faceless one
After 1.5 seconds your faceless one if engulfed in power and becomes a C'Thrax. As a C'Thrax it has 20% increased damage and health and also gains 3 abilities. 20 Second Cooldown (Starts after the faceless one has spawned)
Q- Surge of Darkness: Empower your C'Thrax, granting it double physical damage for 5 seconds, but reduce it's movement speed by 30%. 20 second cooldown. W- Aura Of Dispare: Prevent Mana gain and reduce spell power by 10% in an area around the C'Thrax. No cooldown, costs 60 health per second. E- Mark of the Faceless: Target an enemy hero, gain 10 armour and siphon 50 (+4 per level) health per second for 10 seconds. 6 second cooldown (starts after the previous has ended)

Control Mantid Warrior
Mantid warriors spawn from your core, the Mantid warrior has 1400 (+4% per level) health and deals 100 (+4 per level) damage. The Mantid has 110% speed, and a 20% life steal.
Only one Mantid can be active at a time, with each automatically being re-summoned 20 seconds after the last has died.
When a Mantid is alive it can be toggled to control the Mantid. While in control of a Mantid gain 2 new abilities.
Q- Bulwark: Grants your mantid 50 armour for 4 seconds. 20 second cooldown E- Dread Screech: Your Mantid lets out a screech, dealing 60 (+4 per level) damage and silencing nearby enemies for 1 second. 10 second cooldown.

Summon Sha
Corrupt an enemy minion, Replacing it with a random Sha. (20 Second Cooldown)
Up to 4 Sha can be active at a time (only one type at a time), Sha will follow a random friendly hero and will attack whatever they are attacking.
Sha have 800 (+4% per level ) health and 120 (+4% per level) damage.
Sha of doubt: Enemies attacked by this Sha will use a random basic ability on the nearest appropriate target. 20 second cooldown
Sha of despair: Enemies attacked by this Sha will deal 20% less damage for 5 seconds. 15 Second cooldown
Sha of fear: Enemies attacked by this Sha will be feared for .75 second. 20 second cooldown
Sha of hatred: Enemies attacked by this Sha will deal 15% more damage, but will take 20% more damage. lasts 5 seconds. 15 second cooldown
Sha of anger: Enemies attacked by this Sha will be taunted by the nearest hero for 1 second, friendly or otherwise. (This allows enemy teammates to damage eachother). 30 second cooldown

Elemental Acendant
Warp a friendly minion into an Fire Elemental ascendant, the Fire ascendant has 1000 (+4% per level) health and 140 (+4% per level) damage. Lasts 12 second (40 second cooldown (starts immediately)
The Fire ascendant will leave behind them a trail of embers that lasts for 6 seconds and deals 30 (+4% per level) damage per second to enemies which walk over it. The elemental can send out a blast of magma at a target which reduces slows them by 10% for 1 second and deals 200 (+4% per level) damage once every 3 second.

Fire bolts of powerful energy randomly at all enemy heroes and forts (excluding walls, higher chance of hitting a hero than a structure.). Each bolt does 400 damage, starts with 4 bolts. 200 second cooldown

Eternal Servants
Resurrect all dead heroes. Dead heroes have half health, cannot use heroics, give 25% exp when killed and last for 20 seconds. 120 second cooldown

Blood of the ancient one
Select a visible area on the map. After 1.25 seconds a pool of the ancient one's blood will seep from the ground to deal 150 (+4% per level) damage per second for 8 seconds. 60 second cooldown. (Think of an area the size of a boss stun.)

Kai'ju
Summon Gahz'rilla, to wreck havoc upon the nexus. Gahz'rilla has 1200 (+4% per level) Health, 15 spell armour and 130 (+4 per level) Damage. 100 second cooldown, starts when Gahz'rilla is killed.
Gahz'rilla has 3 abilities: Q- Ice Spikes: Ice Spikes rise from the ground, dealing 200 (+4 per level) damage and knocks enemies back. 12 second cooldown W- Slam: Slam your body to the ground, dealing 100 (+4 per level) damage and stunning enemies for 0.75 seconds. 12 second cooldown E- Frost Breath: Target an enemy hero and freeze them, putting them into a stasis for 3 seconds, during this time they will take 60 (+4% per level) damage per second, but cannot take any external damage. 70 second cooldown

1) Chaotic regeneration (D): You now regenerate up to 70% of your health and have full immunity to percentage damage. 2) Self Imprisonment (D): Bury yourself underneath the ground for 3 seconds, during this time you cannot be damaged, cannot used abilities and tentacles are no longer summoned. 120 second cooldown 3) Heretics! (D): For 10 seconds you now summon 6 tentacles rather than 3, and they deal 30% more damage and reach 1 unit further. 80 second cooldown
Level 20:
1.1) Recurring Nightmare (R): Whenever a bolt hits a hero, summon a random Qiraji at their location. 1.2) He meant it literally (R): If a resurrected friendly hero survives the full 20 seconds, spawn the real hero at the current location with full health. 1.3) The Ancient One (R): Every time an enemy hero is damaged by botAO increase it's duration by .3 seconds, if an enemy hero is killed by botAO remove the blood pool and summon the ancient one.
The ancient one has 1500 health and deals 500 damage. (attacks 1 time per second). 1.4) Five heads are better than three (R): Gahz'rilla now inflicts -15% healing on heroes attacked and deals 33% more damage 2: 3:
