She's not been free to play since I've been here, but from trying her and from video guides I've seen it looks like you need to spam Q all the time to get the best out of her, is this right?

Short answer: Yes. If you notice her talents, most of them improve Withering Fire or have synergy with it. Overall, you want to use Withering Fire as much as possible, but you should not be doing it thoughtlessly. How you use her Q effectively will depend on how you build her, particularly whether or not you take Remorseless at level 7. When you take Remorseless, you should be weaving Basic Attacks in between Withering Fires to maximize your damage output. Without Remorseless, you still want to make sure your Withering Fire continues to lock-on to a single target. Withering Fire does little damage, and when it spreads the damage is often negligible.
