Damn, that was a lot to write and I never addressed 2-lane maps (mines, new heaven/hell map). I've used this build effectively on every map, and feel that it is equally effective on every map. I will update with specifics on certain maps in the next few days.
Two things too important to wait though:
On haunted mines, the nature of the objective requires the enemy to essentially leave the map to enter the mines. This is great, since it makes responding to your pushes much more costly for them in terms of time and team fights in the mines.
On cursed hollows, the 3 carpace talents go a long way towards negating the benefit conferred by the curse. You can toss it on all 3 waves of minions just as they leave the core, and by the time they meet the enemy wave they should be mostly healed. You can of course chain-carpace to keep them perpetually healed (which I should mention is also incredibly useful on an allied hero who's being focused down).
